Terms and conditions
To comply with the new EU directive (Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013) the following information must be made clear before an order takes place.

The following is legally required for distance selling on all web sites:
The total price of our goods, services and digital content are clearly displayed on the order page. Prices include delivery charges where applicable.
Additional data entry charges: Where data entry is involved, the number of responses included in the price are up to 45 patient and up to 20 colleagues (colleague survey is usually completed online). Any additional data entry is chargable at £0.80 (80 pence) per questionnaire. (e.g. if you supply 55 patient questionnaires, 45 would be included in the price, and 10 would need to be paid for at an additional £8). We will contact you for further instruction as to how you wish to proceed, if additional data entry is required.
Payment can be made via credit card, debit card, PayPal, cheque, bank transfer or over the telephone.
The time until we send you reports cannot be estimated, as it depends on the return of completed patient questionnaires to DrMSF and the time taken for colleagues to respond. The current average is between 4 and 6 weeks from ordering to provision of reports. We will despatch the goods after purchase where applicable, we post via Royal Mail First Class, please contact us for an estimate of delivery time.
All warranties, conditions and other terms implied by statute or common law are, to the fullest extent permitted by law, excluded from this Agreement. Dr J. R. Davies shall not under any circumstances whatever be liable for: loss of profits or any special, indirect or consequential loss, costs, damages, charges or expenses.
DrMSF values your custom. We will work hard to make sure you are satisfied with our service. If you have experienced a problem then please contact us, we are reasonable.
DrMSF are happy to offer a full refund at any point before the supply of services has begun, minus a card processing fee of 4.5% (fee we have to pay to process and refund transactions). If the supply of services has begun, then please contact us. Where the supply of services has begun, a partial refund may be offered at our discretion.
If you want to cancel, please contact us as soon as possible in writing or by email, and always within 14 days of receipt of any items. To meet the withdrawal deadline of 14 days, it is sufficient for you to send your communication concerning your exercise of the right of withdrawal before the withdrawal period has expired.
The Consumer Rights Directive right to cancel does not appy to the supply of goods that are made to the customer's specification or are personalised. Our products and services are mostly bespoke and completed to your specification. For example, we print your names on patient questionnaires, set up personalised online surveys, invite your nominated colleagues, and complete data entry and analysis before producing reports for your questionnaires.
If you wish to return goods that have not been personalised, a refund can be given if you inform us by an unequivocal statement (a letter sent by fax or an email) within 14 days of receipt of the items. You will have to bear the direct cost of returning the goods. As an indication of cost of return, currently the full pack costs £5.60 to post within the UK using Royal Mail first class service (correct price on 21 May 2014, if using similar packaging to DrMSF).
Password Protection: The password is not specific to you, and is only intended to secure the report in your email archives. DrMSF recommend keeping your report secure and changing the password yourself (please act accordingly to your beliefs about the sensitivity of the data, and your perception of how useful it would be to a third party).
